Find Recurring data based on two criteria

I am struggling with Mode and Aggregate formulas to no avail.

I am working on a report that I need to find the most common reoccurring value based on two criteria.

The reoccurring column is failure codes, which as my title suggests may contain several duplicates.

The criteria I need to reference is the widget part number and the manufacturing year, each are in two different columns.

Any help on how to handle this as one formula?

Please attach a sample workbook (not a picture or pasted copy). Make sure there is just enough data to demonstrate your need. Include a BEFORE sheet and an AFTER sheet in the workbook if needed to show the process you're trying to complete or automate. Make sure your desired results are shown, mock them up manually if necessary.

Remember to desensitize the data.

Click on GO ADVANCED and then scroll down to Manage Attachments to open the upload window.
