Reading values from table to plot on graph

Hello guys,

I am working on an assignment, am trying to display the corresponding number/value on a cell based on a table. To understand this better please see the attached sheet.

Here is an example of what I am trying to achieve:

In the attached Image, you will see there is a graph I am plotting from a table. The number(s) are displayed from the table of the conditions for severity and probability is met. Right now I entered the values manually. If anyone can help me with possible formula or if statements that will display these numbers and also separate each value with a comma or space it will be of great help.

I have attached the excel sheet as well, please let me know if more information is needed.
