Multiple column index search with different match conditions

Dear all,

Im using and index function in combination with match function, I have a 3 criteria in columns and 1 row. The issue is that 1 of the column is calcaluted percentag which I cannot write accuretely to "look for" field, therefore I tried to set match condition -1, but it does not give me back correct results, the other 2 columns are A) a number in range 1 to 3 and B) simple text. Is there anyway how to resolve the issue?

The formula is: INDEX(G40:FU768;MATCH(D2&"_"&C2&"_"&E2;F40:F768&"_"&D40:D768&"_"&A40:A768;-1);MATCH(B2;G39:FU39;0))

Post a small Excel sheet (not a picture) showing realistic & representative sample data WITHOUT confidential information (10-20 rows, not thousands...) and some manually calculated results. Just before posting, scroll down to GO ADVANCED, click, and then scroll down to MANAGE ATTACHMENTS and click again. Now follow the instructions at the top of that screen.
