If And with Index and Match ?

Hello All

Hoping this makes sense. I am not sure if I need an If/And or an index and match formula but this is what I am looking to match the # in Column A2 and the corresponding date in C2 with the # in Column I and date in Column M and populate Column D with the text in Column N. I have color coded the attached example to make the explanation a bit more clear.
