tally up the numbers each individual has accepted or declined, month by month

Hi,

Attached is my data set, I'd like to tally up the numbers each individual has accepted or declined, month by month.

Id also like to set prompts when they accept a more than say 6 shifts within a month.

The data is being pulled from MS Forms and have created a flow into this spreadsheet table.

Tell us what you are trying to do, not how you think it should be done. "VLOOKUP Help" tells us nothing, and in your ase, looks as if it's entirely the wrong function, anyway.
