Find a value in a table and return a value from the left column

I have a form that in the first sheet calculates according to personal
identification number the age of the person, the age group according to the age and whether the person is a man or a woman. The second sheet contains a table with the standards from which the data are taken. The third sheet is a form that must evaluate the result. The attachment is for a person who is a woman and is a third age group. In this case, you must find an approximate or exact result in the standard sheet in column 7 (E2 * 2 + J2) and show the score from the first column of the standard sheet in column "H" of the form sheet. In the attachment in the form sheet in column F I gave a result that should display an estimate as I described in column "H". Thanks.

This would be a lot easier if the lookup table were normalised (redesigned). Is this a possibility?

I was thinking about this option to turn the table over and use LOOKUP, but the table is much bigger in reality.

All the more reason to redesign it! The layout is not at all good for looking things up, and you should NEVER EVER use merged cells in lookup data.

I won't be helping based on the fact that the sample data is not truly representative, so any suggestion is unlikely to work when applied to the real data. Good luck.
