Power Query - Present Data from Colom to Row

Hi,
I am trying to learn how to user Power Query.

What function do we use to convert some of the information from Column to Row and then combining them into one list. I have attach a sample file as the original file is very very big. Thanks Expert.

I have update the table with correct out put arrangement and PowerPoint image on the concept on how the current data is replicated manually. Hope expert can help. Thanks

I'm trying to see how the information in the Output ties to the information in the Input. Some of the data in the output seems to be made up and not linked to the input. Please clarify so that we can propose a viable workable solution that we can test and tie to your output.

I think I may have understood your needs. Look at the attached and see if this is what you meant. It involved appending the table to itself after removing some columns and then unpivoting the data and renaming one column to ensure a smooth append
