Dynamic List with Multiple Criteria

Good Morning,

I wondered if anyone could help me? I have list of data in worksheet "data" where I want to pull information from. I want to be able to enter a unique code and bring up a list of items only if it meets specific criterias. Attached is an example with more explanation.
