Index Match apparent anomaly (re-posted in correct forum)

Hi, I'm creating a table of tasks and want to select the person's name to lead each of the tasks by placing the letter "l" in the column/row which corresponds to their name and task id.

Using an Index Match formula, all seemed well until noticing that some rows weren't working properly as shown in the example attached.

I also use the letter "t" to indicate a team member and have found by removing some of the letter ts in the problematic rows has an impact but don't understand this and I cannot find a solution which retains the functionality of indicating both leader and team member roles.

Could anyone help with this please? Thanks, Jess

The post is not duplicated. I originally posted in the incorrect forum and then corrected this mistake by deleting the original post and adding this one.

To confirm, this is not a duplicated post and assistance with it would be appreciated. Thank you
