Index Match Formula

I have attached example excel of my struggles.

If B2 is June then A B C and D should return the values as follows.

Data pulls from Sheet 2 to Sheet 1

I am receieving #N/A error when I try this

It is giving me #SPILL when I try and input this into Sheet 1 B5 cell
