Using V&H LOOKUP With MATCH to find a value.

So, I'm not sure how to explain this, but I've been trying to use vlookup, hlookup, and match to grab information from the correct row.

I have a column with duplicate values and cannot seem to figure out how to differentiate between them.

    Any help is greatly appreciated.
