    match/index formula help

    Im trying to look up data on one worksheet using a match and index formula. the problem Im running into is that the formula is returning dupicate values on multiple lines. Im uploading an example workbook to try and help show what im talking about. Any help would be greatly appreciated.

    CSE, aka Array aka { }, formulae are confirmed with CONTROL+SHIFT+ENTER.
    Replace commas ( , ) with semicolons ( ; ) in formulae, if your locale setting demands.
