Help with INDEX MATCH embeded functions with 2 way lookups

I am using the index match functions, along with an error check, to look through data and find 2 things.
1. To match the Port ID to the data set.
2. To match the Location ID to the data set.

I am dumping a lot of data into a tab and what I am trying to accomplish is have the formulas search to see if the Port ID exists for a certain Location ID.
If it does, then return the Month to Date (MTD) #, if it does not then return 0. That seems to work.

The issue I run into is, the formulas do not pull the data correctly in ever scenario.
For example, I attached spreadsheet. Port ID 460000 doesn't pull thorugh for Location ID 1, instead it puts it under location ID 2.
And it seems that nothing pulled through for location ID 2 at all.
I've tried this a few different ways with different types of lookups and/or multiple Index Match statements, and I can't find the prefect solution.

I actually have 20 location ID's to work through. In the example attached, I am just using 2.
Any ideas or suggestions would be greatly appreciated.
