I have data in following format (for all employees).

I would like to have employee-wise report in another worksheet...somting like below:
Employee Salary History Report

Name of Employee
Effective Date Type GradeBefore GradeAfter OldBasic NewBasic
Can you please help in creating this report. I have attached the MS Excel file in which I have compiled the data.
