    combine if & vlookup

    I need help with the following problem:

    I have an excel spreadsheet with one worksheet containing a list of fees & codes associated with different insurance companies. Ex:
    Code Procedure 2012 Fee Aetna Ameritas
    D0150 OV- New Pt. Exam 85 35 45
    99201 OV- New Pt. Exam 85 35 45

    each in different columns. My other worksheet needs to pull a code from this worksheet but have it be determined by what insurance is put in.
    Insurance Code Procedure
    (they will put Aetna here) (and a code here) I need a formula here that will pull corresponding fee for this code & this insurance

    Now able to guess what should be there under the columns [Code Procedure 2012 Fee Aetna Ameritas]..
    please upload a sample file..
    If you want to do it your own.. try looking at Index- match combinations... with array functionality.. thanks.
