Index Match based on 2 Criteria only returning 1 match

I am trying to pull all the products from a sheet called pet historical all channels, that ran promos in NOV 13, and are in the DRUG channel. When I enter this formula, hit CTRL-SHFT-ENTER, and copy down, I only return one product (the same product) on every row. I used this formula with 3 criteria on another sheet and it worked fine. Not sure what I am doing wrong. Thanks for any help.

    =INDEX('pet historical all channels'!$Q$2:$Q$65536,MATCH("NOV 13",'pet historical all channels'!$H$2:$H$65536,0),MATCH("DRUG",'pet historical all channels'!$J$2:$J$65536,0))

This is a simplified version, the formulas are in the yellow cells. I hit Ctrl Shift Enter and dragged down.
