formula to move data from multiple columns to rows

I'm creating a import template to another system from an existing excel report. The existing excel report includes a list of employee names in rows with hours split across multiple columns however, the import template requires the data to be imported by row which will requrie multiple rows for each employee name. Is there a formula I can use to take the data from columns, populate it as rows and repeat the common data (name, number etc) for each row of unique data? I saw a thread using index formulas but couldn't quite get it work for me. I've attached the workbook, it has minimal data for testing, in a completed report there could be 30 columns and 150-200 rows.

    thanks

    Hi emexpress,

    Welcome to the forum.

I guess you need formulas on "import template" sheet which will fetch data from "form" sheet.. but on "form" sheet, I am not able to find a employee with multiple rows.. can you create few more rows and highlight your expected output so that it gets confirmed. thanks.

Thanks for replying. I have updated the attachment to add a worksheet showing the expected output. You will see that the expected output is taking the two employee rows on the form tab and listing the unique data in columns beginning at col. U for that employee. On the expected output sheet I have highlighted the column headings in yellow for those that are the unique data coming from col. U and right while the other columns of data from col A-T on the form sheet will repeat thus creating as many rows for each employee that there is unique values entered on the form for that employee beginning in col. U. Hope this helps explain.
