Index and Match Question

Dear All:

I am trying to display a value from a different work sheet using both index and match functions and am having difficulty.

My objective is to display the date of surgery that a patient had, if they had a given surgery. The raw surgical data is in the "raw" worksheet. If they had a surgery of interest, then it will show up as a 1 in columns D-L. Depending on the order of their surgery(1st, 2nd, etc), I would like the date to show up in the corresponding column in the "cumulative" worksheet(Column C,E,I, L, etc).

The CPT corresponding to the surgery of interest are listed below:
URS: 52336, 52337, 52352, 52353
ESWL: 50590
PCNL: 50080, 50081, 50553, 50561, 50551, 50570, 50580, 50557, 50572, 50576
Open Pyelolithotomy: 50610, 50620, 50630, 50065, 50075, 50060, 50130
Laparoscopic Procedure: 50945, 49320, 50549
Nephrectomy: 50546, 50220, 50225, 50548, 50230, 50240

*As a caveat, the file that I have attached is incomplete due to size restrictions. The unique numbers in the "raw" worksheet are incomplete and some of the values in the cumulative data may not show up in the raw data
