Duplicate values returned using Index Match Lookup with Small()

Greetings All

I am trying to build a set of dashboard Top 10 and Bottom 10 charts and used the Large() and Small() functions to select the top and bottom sets of values. So far, so good, but then in order to put names with the values, I used an Index(Match()) combination and I have 3 values in my sample data that are the same but have different Names associated with them.

I feel as though there must be a way to get the unique names associated with the duplicated values but I am evidently missing something.

Any help would be greatly appreciated.
