Time Sheet (Tie in extra spreadsheet to existing spreadsheet with formulas)

Ok, almost mission complete on the timesheet. I have learned much and I appreciate you support. Attached timesheet has Tab "PTO Calendar Week 1-4' already tied to "PTO Rqst. Form with hyperlink formulas. I added "PTO Calendar Week 5-8" and am trying to get the same formulas that "PTO Calendar Week 1-4" has to duplicate on "PTO Calendar Week 5-8" The dates filled in “yellow fill" reflected on the "PTO Rqst. Form are dates that come from "The PTO Calendar weeks". trying to get calendar week 5-8 to also populate the "yellow fill" dates on the "pto reqst. form AND if possible i am trying to get the dates from the pto reqst. form to reflect on the PTO calendar week 1-4 "in red fill"? I know it’s allot? The dates currently show up as numbers on the pto reqst. form (i.e 3/3, month then day), I would like to see if can reflect with the abbreviated month in text format and then the day (i.e. Mar 3)...
