Showing combine value in Pivot Table

i have table of Five different columns and when I try create Pivot of that table I want to show it like combine of first two columns in ROW LABEL.
I have also attached screen shot and excel file of that.
ANy help would be appreciated.

Create a helper column and use that column for your pivot table

i want to show it without using Helper column...
Just need to ask cant we do this by using Calcutaled Item.
