Formula that will provide a text result from 3 different formulas

Hello Excel experts, I need your assistance. I seem to have forgotten how to write formulas today. I need to create unique identifier from existing data in the spreadsheet as follows: The month the individual was born, the day they were born, the year they were born, the first three letters of their last name and the first letter of their first name. I have the individual formulas, but am having quite the time getting them combined without an error message.
Cells:
A2: Last Name (example: Smith)
B2: First Name (example: John)
H2: Date of Birth (07-04-1976)

Birthdate formula: =(text(H2,"MMDDYY"))
First 3 of Last name: =UPPER(LEFT(A2,3))
First Letter of First Name: =UPPER(LEFT(B2,1))

Result should be as follows: 070476SMIJ

How do I combine them into one cohesive formula? Please help.
