Indirect function to pull desired no of cells/row data based on data validation in Input

Dear Sir,

As enclosed workbook for your kind reference, my problem is that I want to pull data in sheet 'Output!B5' onwards based on the condition of Output!C1,Output!C2 & Output!C3:F3.There is data validation in 'Input 'Sheet in C2 & C3 and upon changing corresponding data in Output sheet changes from C1:F3.

Here,if condition of Output!C3:F3 is not blank than how to readjust my formula in Output!B5 onwards,so that there would be unique name in Output!B5 onwards cells/rows and no duplicates.But if in Output!C3 is not blank than Output!B5 onwards should display only data that is of Output!C3 from the specific sheet that is reflecting in Output!C1 cell.In summary,

If Output!C3 is only visible >>Then >>Output!B5 onwards cells/row display only that contains Output!C3 data from specific sheet mentioned in Output!C1
If Output!D3 is only visible >>Then >>Output!B5 onwards cells/row display only that contains Output!D3 data from specific sheet mentioned in Output!C1
If Output!E3 is only visible >>Then >>Output!B5 onwards cells/row display only that contains Output!E3 data from specific sheet mentioned in Output!C1
If Output!F3 is only visible >>Then >>Output!B5 onwards cells/row display only that contains Output!F3 data from specific sheet mentioned in Output!C1
If Output!C3:F3 is visible >>Then >>Output!B5 onwards cells/row display only the unique values with no duplicates that contains Output!C3:F3 data from specific sheet mentioned in Output!C1
