Is excel can generate form?

Hi everyone, I am guessing here if excel can do this trick..please do check my attached file.

As you can see I have only one attached file which has a two sheet tab, one is the "DATALIST" and the other is "FORM" in which this tab will be the one to be
generated with blank highlighted yellow, and get data from datalist tab..... I am thinking of this because i have so many data to be inputted in this form and very time consuming I am just wondering if excel can do this trick...

Ok here it is I inserted unique_id's

in the form tab, the member id change it to unique id....

It can be the "call" of school...
for example ;
I inputted Name of School: TAPILON NATIONAL HIGH SCHOOL, automatically the facility ID, municipality and province will appear also and
All Students who in school in TAPILON NATIONAL HIGH SCHOOL will be added to the form at the same time the rows data will be added to their respected name including the following: household_ID, member_ID, last name, first name, middle name, extension name, gender, birthdate, age and grade level codes.

I hope...this explains for good...
