    Exclamation HELP/RUSH: Excel Appointment Scheduler

    Greetings to all,

I NEED YOUR HELP. Any volunteers for this crazy project of mine? Please

I'm basically new to this forum and I'm seeking help urgently with a project. I am trying to create an...
"Automatic-perpetual Appointment Scheduler". This document will help me see the appointments that have been coordinated for each doctor on each day of a week. Now, I have NO idea where to start, how to do it, but I do know what I would like to have on that document. This following model that I will show, is a free template from an amazing creator named Randy Austin, you can find him on youtube.

Here is an example of what I would like, but far less complicated I think.
    Screen Shot 2014-04-25 at 8.34.44 PM.png

I am looking for the following features:
    1. Weekly view (that would fit my excel screen and I wouldn't have to drag too much with my mouse), for each doctor (4 doctors with their names on top like the picture) and hours from 8 am to 6 pm with intervals of 30 minutes.
    2. In the area that says "Type" and "Contact name" (as in the picture), I would like "City" and "Patient Name".
    3. I would like the calendar to be perpetual and automatic; that means, to have a button such as a spin button to change the weeks and make them perpetual, so it could go on and on and on.
    4. On top of the dates. I would like to have a spaces to write the information. For example, leave like 4 to 5 rows in blank for this.. What I mean is: Instead of typing the city and name of the patient on the same "cell" in the scheduler plan, and instead of just typing the patient's name and city under the doctor's cell, I would like to have a space in where I could write the name and the city manually, and then with a scroll down box selected 1 of the four doctors. And when finished, just click a label to submit the information, that after pressing it would appear and reflect on the interactive weekly calendar.

That's really all, I know it's a bit too much maybe. I have no idea how complicated this is, I've been trying to read and watch videos and try, but I failed every single time. I need someone who would like to teach or have patience or would like to help me out. This project is for my work and I was supposed to have it for monday. Now, I have no idea what to do so I am requesting help.

    What you guys think?
