Intriguing error regarding index/match functions

I was looking at the other member's post,

and came across an unexpected error, with reference to the attached,

Vlookup in Column C and Index/Match in Column D should get the same results but I just can't figured out the #N/A in D4!
Could someone shed some light on it please.

you are missing the final parameter for exact match
=INDEX($B$9:$B$12,MATCH(B2,$A$9:$A$12,0))
