Match year of date to year of date specific range of dates, output corresponding value.

Hi All,

Basically what I'm trying to do is the following:

Match the year of a specific date to the year in a range of dates and then output a value that corresponds with this date.

I have attached a sample spreadsheet and screenshot which shows what I am looking to do.
