if a range of cell is empty then need the result ok,otherwise not ok

pls help to get result like below (file is also attached)

WHEN
APP ISSUE W-SHADE(4 CLR)=OK RESULT SHOULD BE LIKE THIS
SUB DUE
COM DUE
APP RED & BLUE&BLACK&WHITE
