Please help to get the output in attached excel. The logic is : If column C2 and D2 both are "Yes" then output value two values for the same one referring FY15/16 and other as FY16/17 and if only C2 is "Yes" then output should have one value. Output should not have duplicate values.
What is the output supposed to be?

You supplied an example, but did not supply what the result should be
