Help Designing Lookup Formula Using 2 Inputs, Returning N Found Results

Above is the spreadsheet I'm working on.

What I want to do is develop a formula that will look at the PG Distribution List worksheet, perform a search on that sheet based on the first two columns (practice group and employee role), and return all values in the C,D and E columns where A and B match the input.

I cannot figure it out and I've tried creating Index/Match searches but I just don't have the expertise to really perform the search.
