v-look up inquiry

Hello,

I'm trying to do a look up from tab 2 (meter list) and populate the info into tab 1 (Dec), for some reason the highlighted items are not picking up the correct information. I know this has to due with duplicate values but not sure how to correct?

I have attached a copy of my spreadsheetSS.xlsx
