Output from multiple fields - formula

Hello,

I'm trying to output based on whether something is active and labeled social.

Clients Active? Social
Client A Yes Yes
Client B No Yes
Client C Yes Yes
Client D Yes No

So the output would be Client A & Client C each being in their own row.

I tried an index, match formula which got close but didn't get all the way there.

Any help is appreciated.
