Need help with generating a formula

Dear guys, I need a help with coming up with an excel formula. I am using Excel 2010. So here is what I have:

I have 5 columns with the following headers - Company name, year, Total assets, Total debt, Total equity and many rows with observations.

I need to do the following for each year: Firms were defined as issuing (repurchasing)a security when the net amount issued (repurchased) divided by the book value of assets exceeded 5%. For example, a firm was defined as issuing equity when (equity issued-equityretired)/assets > 5%. Cases where firms issued (repurchased) both debt and equity in a given fiscal year are omitted.

I have all the data but don`t know how to do it. I tried with the countif, didnt work. The result has to be like this:
