Form for tracking item updates

Hi all,

At work, I have to keep track of visual effect shots (for film/tv). The shots have unique names, eg NAM_001_010, NAM_001_020,
where the first 3 digits are scene number and the second 3 numbers are the shots in the scene that require effect work.

As the shots get worked on, I am sent multiple versions of the same shot.
For example, on June 1, I receive NAM_001_010_v01, on June 6, I might receive NAM_001_010_v05. The v01, v05 indicates the version of the effect.
Other shot specific information needed is the time code in and out points (HH:MM:SS:FF) and which reel of the film the shot is in.

I am looking to create a form that I can use to enter all the shot names and then when I receive a shot, call up the effect shot and enter the date and version number received.

I also want to be able to indicate the name of the company that delivered the shot and, when a shot has been approved, have some way of indicating the version that was approved - which isn't always the last one sent.

I have searched through some excel templates and haven't quite found anything that I could adapt for my specific use and not being a coding expert, I was wondering if anyone could point me in the right direction
before I start something I know will lead to long, sleepless nights.
