Hi there good people. I'm sooo excited about this survey function - maybe I need to get out more!!!

I'm very new to Excel so probably a very simple solution that I've overlooked but need help with please.

I've created a very basic survey that asks me for my lung function (I have cystic fibrosis) so that when I do a reading throughout the day I can click on the survey and fill it out. It then saves the data to the excel file so that I can send it on to my hospital.

The input fields are date, time & reading.

My main question is how can I share this with myself so that I can put it on my desk top machine & my mobile phone in onedrive so that I just click, fill out the form & close?

Other ancillary questions because of my ignorance at the mo are:

Date - I've set this to required, but is there a way of it auto filling if I leave it blank with the current day?
Time - I have to use the colon format but wondered if there's a way to just enter a 2 digit number to the nearest hour so that I don't have to faff with the colon all the time? So if it's 15:37 I could just type 16 for 4pm.

Thank you kindly for looking at this & hopefully you may be able to steer me along the right path as everything I've surfed for thus far hasn't really applied!

ps I'd like to eventually end up with about 6 survey links on my devices covering, lung function, weight, temperature, BM's (I'm diabetic too), O2 saturations, etc. Be a godsend to be able to click & record.
