Conditional formatting for icon sets is not working properly

Hi,

I am trying to apply conditional formatting for icon sets. For some reason I do not get it working despite of very intuitive interface for it. Please see the attached file and intended value ranges for different colors.

Also is it possible to apply conditional formatting through icon sets for single cells? For the row "Completion rate_YTD" I want to assign individual ranges.
