This is not quite the design you specified, but it does accomplish the requirement, I believe.
The last sheet in the workbook contains the Clinic Table. I filled it in with the first couple of clinics. Complete filling it in and as you add clinics here, they get added to the dropdown list on the Clinics sheet. Once you have this table filled in, you can hide the sheet until you need to unhide it to add new clinics.
The next to last sheet is called Template. This is basically the “Blank” sheet for data entry. This sheet can be hidden as well.
On the Clinic Sheet, select the clinic you desire and click on the Go to Clinic Button. This button will take you to the desired sheet if it exists. If it does not exist, it will ask you if you want to create it.
Or you can click on the Create Clinic Button. This button creates the sheet if it does not already exist. If the sheet does exist, it will give you a message and take you there.
On the individual sheets. When you click on the Clinic Tab Button, it opens the Clinic Summary sheet, finds the clinic name there and copies the Monthly CPT data to that row.

A picture is worth a thousand words, but a sample spreadsheet is more likely to be worked on.
When I create a new clinic the clinic name does not change and the clinic information does not populate on the clinic summary tab. how would I do this?
What do you mean by, "the clinic name does not change?" You can't select it in the dropdown box? Or when you press the button it does not go to or create a page with that name?
When I create a new clinic it does populate a new tab but the clinic information does not populate on the clinic summary tab on a new line.
OK, I am able to duplicate the issue. I think it is a simple fix. I will get to it later today.
Fixed - I believe.
