So Cell A is a drop down List with two choices "extra" or "not extra". Cell B tells me what i need to cut the steel at but if it is "Extra" it needs to be +44 if Cell A says "not extra" Cell B just give me the normal cut length.

depends how is cell b populated?
if you manually have to type it in...suggest cell C to calculate B + (output of A)
if it draws from something else then you can simply add calculation to additionally add

Currently Cell B is populated with a Cell reference of L10 giving me at this very moment 1100mm, but if Cell A says "Extra" it has to be 1144mm
So in B1:
=IF(A1="Extra","1114mm",L10)
