Auto Populate data from an Existing Sheet

Dear Experts,

Please help in the attached sheet. I have a set of data, which needs to get auto populated in another sheet. I tried with Index and Match function but getting error.

First thing don't merge the cells and put the each header. Second make sure you month format should always same in both sheet. then try
