Unable to get Icon file when embedding a file in Excel

Excel version: 2010
OS: Windows 7
A co-worker is having a problem with icons when she tries to embed a file in Excel 2010. We have to routinely embed files in Excel when preparing reports for customers (mostly PDF files).
We both go Insert>Object then Create from File. Then Browse to locate the file and check Display as Icon. The problem appears when she tries to change the icon.
She gets this Icon ChrisIcon.JPG. I know it looks ok, but it changed. But there's more. When she clicks on Change Icons, there are no options available, not even the one shown. When she accepts the icon it does not appear on the embedded file, instead she get some kind of hexadecimal code (sorry I don't have an image of that). I gave her the file path for the PDF and Excel icons, but that doesn't work either. Everything worked fine until she tool her laptop on a business trip. Any ideas?

• change icon
• path: c:\windows\system32\shell32.dll
• select icon
• select file

and so on...

if you want change icon again you need repeat these steps from above
