    Welcome to the Forum (Bienvenue)!

It appears that you forgot to attach your file. The paper clip icon does not work for attachments. Instead, under the text box where you type your reply click the Go Advanced button. On the next screen scroll down and click on Manage Attachments, which will show a pop-up window to Select and Upload a file. Then close the window.

    Re: Form compilation

There is not simply one function that you need.

First you must re-create the form in Excel. You can do this with cell formatting and adjusting column widths.

The following fields will probably require some kind of function or formula, but you will need to explain what these mean if we are going to help:

    hours at 150% 42.5
    hours at 200% 0
    Total Hour 100% 103.75

Once that is done, you need to show us what you want your table to look like, and what kind of statistics you want to show.
