    Build your own userform rather than use the automatic one.

    You can design your own custom forms within the VB Editor (Press Alt +F11 to open this).
    It will involve some knowledge of VBA so if you don't have that skill attach a sample workbook and a detailed description of what you need the form to do and we can help you from there.
