    Customer data multiple rows into 1 row

    I have attached a sample data set of what I am looking at. I have customer service data listed on multiple rows beneath the main customer data row. I need all of this data on 1 line for my use. The service data is inconsistent on the # of rows, anywhere from 1 row to 4 rows. I would like the red text on to be transferred to the highlighted yellow cells. I tried using the index/match function but the formula gets thrown off because of the # of rows of service data. When I have 1 row of service data, my formula will pull data from the next customer.

    Any help is appreciated.

    If you can use vba then this would fill your columns F:H where applicable.
