How to track changes made to data and have user put in reason why they changed the data

I'm sorry if this is the wrong location to post this question, please tell me where to post and I will repost it. My question is I have a workbook with data set of set points of mechanical equipment on the field. If we decided to change the set point out on the field we want to change the data in the workbook. But before the data is changed is there a way to have the user (or automatically by excel) be forced to input what the set point was changed from and to, date the user changed the data, their user id, and most importantly the reason why the data was changed.

Ideally excel would be able to calculate what data was changed from what to what, the date, and their user ID, without the user being forced to input it and all they had to really type was the reason the data was changed.

Please any help will be helpful

    Thanks

I'm doing something similar and using this code. As long as the information is in the spreadsheet you can probably modify this to suit your needs

Thank You, I'm not very familiar with VBA. I tried using VBA on a previous application of excel but it turns out it wasnt needed and I got by without it.
Can you explain whats going on and what variables I would likely need to change. My below remedial knowledge of VBA tells me that there isnt a point where the user is prompt or has to input a reason for the change. Any additional help you can provide will be greatly appreciated.

I'm a long way from being an expert. Essentially what is going on is that as cells get changed their original value gets logged automatically on the Tracker worksheet. It tells me who made the change, what cell and worksheet they made the change to and what the new value is. If there is a FUBAR and I need to backtrack to see what happened I can go to the tracker and figure out what the individual was
