Get a form from Excel data

I would like to know if it's possible to get some data from Excel that would be formatted, for example, like in the following example :

    eNgqx.png

And turn it into a form that would look like that, without doing it manually :

I've tried searching I don't know if it's actually possible to do it.

Thanks in advance for your help!

Re: Get a form from Excel data

How would the form be populated from the data?

Would the user select the Numero for the record they wanted?

Any chance you could upload a workbook with some sample data?

Click on GO ADVANCED, scroll down and click Manage Attachments.

Re: Get a form from Excel data

    Thanks for your reply Norie!

    1) How would the form be populated from the data?
    > Like in the second picture, there would be a label containing each column's name and a (kind of) text input next to it containing the corresponding data of the line being formatted as a form. To sum things up, I'm looking to turn each line of a table into a (kind of) form that I could copy in a slide as a picture for example, so it's more readable.

    2) Would the user select the numero for the record they wanted?
    > Could you ask it in another way? I'm sorry but I'm not sure what you mean with that question.

I have added a workbook of that it would look like but it's exactly like in the first picture with a few more lines of data.

2)The Numero field in your data appears to be some sort of identifier.

If it is it could be used to pull data from the data table to the form..
