I have a form of 4 cells (as you can see in the attached picture) & I need to get the values within brackets of the sentence filled when updating data in A2, B2, C2 & D2.

The code works perfect! However, there was a small problem. Highlighted areas in the attached picture are dates & I need them to come up as dates in the sentence. It would be great if you could help me with this.
