Create excel template to capture data

Hi All,

I am looking to create an excel template where there will be five icons. I want to assign numbers 1,2,3,4,5 respectively. When the user clicks on the image (like push button) it should store the assigned value in another workbook (not same template). The data stored in the another workbook has three headers - Date, Name, Factor; where the Factor is the assigned value.

If this template is used by a 10 member team, then all the records are stored in the non template workbook for analysis.
Is this doable in Excel?

I have a beginner level understanding. I believe using forms we can do this but not sure how? I did spent some time on this too.
