    Multiple criteria formula needed

    Haven't spent much time on Excel in a very long time, wasn't really good at formulas to begin with, but right now feel like I am back at Day 1 with no idea...

    Looking for a simple formula - Based on a date and employee ID#, need to pull the type of leave they took.
