Thanks it works for one requirement but got #Value for other field (Green, expected result)
Can you help me with index match formula? because sometimes I move the result sheet and got N/A if I used Vlookup formula

What do you mean by "move the result sheet"? Post a sample showing the problem....

Sometimes the result sheet, I moved to other my worksheet and it's got #value if I used vlookup formula. But not #value if use Match-Index Formula

I already attached my question above, and I tried to explain to you that I prefer Index-Match formula than vlookup
And I asked you to post a sheet showing the problem... twice...
Here is an Index-Match version of the same. I have no idea what it will do if you "move your other worksheet".
