SMALL and IF statements to return value

Hi,

I have been playing with a formula I found online to return a value in Column A based on specific criteria. It seems to work well for one column but not for multiples.

BUT, when I add to it to look for the same value in column C if there is an error, I don't get a return. If I try the formula separately, I get the return from column C, but not if I add it to the previous one. Do you know what I am doing wrong?
